Please print and complete in its entirety prior to your appointment.

If you are unable to print these forms at home, please arrive approximately 20-30 minutes early to your scheduled appointment time in order to complete them at the office.

Vance Medical is a self-pay clinic, meaning we do not bill insurance, Medicare, or Medicaid. There are a few reasons for this, including:​
  • We would rather have the decision as to what kind of healthcare a patient receives be determined between the doctor and the patient, without a third party intrusion.
  • Almost none of the services we provide are usually covered by insurance.
  • When the insurance company denies payment, we still have to bill the patient. This can sometimes mean collections services, which we would prefer to avoid.
We can (upon request) provide you with a specifically coded, itemized receipt called a “Superbill” that you can submit to your insurance for possible reimbursement. We will not, however, supply these forms for anyone submitting to Medicare, Medicaid, or any supplemental insurance; we apologize for the inconvenience. We understand that this might make it more challenging for some people to see us, and for that we are truly sorry. We hope that this will not deter you from meeting with Dr. Vance and getting the relief that you need.

When are we open?

We are open between the hours of 9:00 am and 5:00 pm on Tuesday, Wednesday and Thursday. We will typically be closed on Monday and Friday but will always have a staff member here to assist you from the hours of 9:00 am and 12:00 pm. If Monday or Friday are the only days that you can be seen, please discuss this with the staff and we will try to accommodate what you need if possible.

Do I have to pay with cash?

Although we are a “cash pay clinic” we don’t actually take any cash as payment for services due to the inability to handle large amounts of cash or make change for patients. We do accept HSA cards, debit cards and charge cards as well as checks from established patients.

Do you take HSA or Medical Savings cards?

Our system accepts all HSA and Medical Savings cards. However, It’s up to the patient to check with their enrolled program and verify that it covers alternative care, supplements, IVs etc.

What about prescriptions, labs, imaging and referrals?

Dr. Vance is a board certified family physician and is able to fill prescriptions, order labs and imaging, and refer you to a specialist if needed. For your convenience, we have an in-office pharmacy with many of the prescription that Dr Vance prescribes for patients.

Can I use insurance for lab work?

Yes, if you have insurance then you may take your lab order to whichever lab your insurance covers and they will bill your insurance. For those patients who don’t have insurance or who may not have met their deductible for the year, we also offer a cash pay option that is much more affordable due to our partnership with both Lab Corp and Treasure Valley Labs.

What about labs if I’m out of state?

We contract with Lab Corp which is a nationwide lab network. If you are paying cash through our office for labs, you would go to your nearest Lab Corp location to have them drawn. If you are paying with insurance, you may take the lab order to any lab that takes your insurance.

Do you offer a cash pay option for lab work?

Vance Medical has a partnership with both Treasure Valley Laboratory (TVL) and Lab Corp. Because of this, our patients may choose to pay us at the clinic and have their labs drawn at one of these two facilities. If you choose to go through insurance and haven’t met your deductible or the insurance doesn’t cover your labs, you may be responsible for the full amount of the labs. Oftentimes, choosing the cash pay option through the clinic is still the cheapest option for patients because of the discounted cost we receive.

Do you have an online portal?

We do not have an online portal, although your appointment reminders will tell you that we do. We apologize but this is a function of the system that we cannot remove. However, if you are in need of any information such as lab reports, imaging reports, notes from your visits, etc. please reach out to us and we will provide you with whatever is needed.

What happens in a new patient visit?

During your new patient visit, Dr. Vance will typically review your most urgent health concerns, past history, any recent labs or imaging reports that you may bring, and discuss treatment options that might be appropriate for you. Depending on your personal health concerns, he may offer treatment right then at the appointment or he may order labs to be reviewed before continuing with the treatment plan.
